How to Claim

There are a couple ways for you to make your claims when dealing with AXA Affin. It varies and depends on the type of claim you are making.

If you require emergency medical attention then you will be driving into the emergency admission at the hospital you visit. In the case that you are at a AXA Affin Panel hospital then you need only present your medical card and let AXA Affin handle the rest. You will be required to fill up a pre-authorization form as well. For the most part this is the easiest method. If you are not at a Panel hospital then you will have to make your own payments and later submit a claim form for reimbursement. 

The next option is that, you might have a scheduled visit to the hospital. In this case, you will need to contact AXA Affin a day or two before your appointment and send them the admission letter provided by the hospital. This way you will receive a guarantee letter from MiCare that you can bring along with you to the appointment, and will make payments easier.

Lastly, if you are requesting a reimbursement from AXA Affin then as per the work flow, you will need to have the claim form that you can get here. You will need to gather a claim statement, medical report, all original bills and receipts and an investigation report. Compile and submit these documents to AXA Affin or your agent and you will have begun the claims process.

Well, the most obvious one is that you will get coverage up to the age of 100. In addition to that, there is high medical coverage with a little perk whether you make a claim or not. With no lifetime limit, no matter what your previous claims are your annual coverage limit remains the same.

Enjoy access to 24-hr assistance for easy cashless admissions at any one of the AXA Affin Panel hospitals whenever you need, in case of an emergency. Lastly, you will get to cash in on some income tax relief for your medical insurance premiums.

How much does it cover?

Aside from all regular hospital expenses like room and board, the use of hospital and surgical equipment, this policy also looks after a number of medical expenses. So you will not have to worry about charges pertaining to surgery and medication or even the use of an ambulance for transportation as they will all be covered and for the most part, repaid in full.

The policy also offers outpatient care in certain instances. You will be covered for physiotherapy treatment, as well as accident and emergency treatment within 31 days from your date of discharge. Over and above that you will have additional limits over your annual limit for kidney dialysis treatment, cancer treatment, accidental death and emergency medical evacuation and repatriation.

There are also a few optional benefits that you can acquire with a little bit extra payment in your premiums. Options include home nursing care, additional lifetime maximum limit for kidney dialysis and cancer treatment, and finally, post hospitalisation and physiotherapy cover extension to 60 days.

Tell me what the policy doesn’t cover.

As with most insurance policies, there are a few exclusions that you need to take note of before purchasing a plan. The policy will not cover hospitalization for investigatory purposes, investigation and treatment for infertility or impotence.

Treatments of sexually transmitted diseases, self-harm, attempted suicide and injuries pertaining to professional sports or dangerous extreme activities are also not included in the plan.

Who can sign up for the policy?

Anyone can apply for this plan as long as you are above the legal age. Parents who want to purchase the policy for their kids can do so as soon as they are more than 15 days old.

The maximum entry age for this plan is 59 years old. If you are a policyholder before then, you can extend the policy renewals till the age of 100.
