To make a claim with Allianz you will need a few things per the list below. But before that you will need to get a claim form to fill in, this will be your ticket to getting your claims approved.

  • Medical Report
  • Original Bills and Receipts
  • Police Report if applicable
  • Copy of NRIC

You will need to compile all these documents along with your claim form, and head on over to the nearest Allianz branch or hand it off to the Agent. You will need to have a copy of your business registration if you are making a claim on behalf of a company.

What is Allianz HealthInsured?

Allianz HealthInsured is a comprehensive medical card by Allianz Life Insurance Malaysia Berhad that you can trust to give you the coverage you will need.

What are the perks of this medical insurance?

One of the perks that come with HealthInsured is that you have NO lifetime limit. You will also have access to a high overall annual limit of up to RM5 million with this policy.

Once you purchase this policy, you will get a medical card for hassle-free admissions. Simply present your card at any panel hospital and let Allianz’s Managed Care Organisation do the rest.

There is an extensive list of panel hospitals that you can check out to find the one closest to you.

During your hospitalisation, your loved ones will be able to request a lodger bed to be placed in your room so that they can care for you overnight.

All lodger expenses will be reimbursed up to the lodger benefit daily limit based on your chosen plan.

Another perk that's worth highlighting is the COVID-19 coverage. Allianz is committed to providing you with the best coverage possible, so if you, unfortunately, tested positive and are under either Category 3, 4 or 5, you'll be covered with this policy. However, this coverage is available only if you're subscribed to Plan 200 and above.

What does this Allianz insurance policy cover?

Like most, this policy covers eligible expenses incurred for hospitalisation, surgery and outpatient treatment. You'll be allowed to claim for pre- and post-hospitalisation medical bills.

This policy also comes with selected outpatient care benefit which includes kidney dialysis if you come down with dengue or typhoid fever.

On top of that, you may also get a second medical opinion should you need them.

This Allianz medical insurance has additionally extended its outpatient cancer coverage to include new treatments such as targeted therapy, hormonal therapy and immunotherapy.

With the extended cancer coverage, you will gain access to genomic testing for cancer which will help doctors identify treatments most suitable for you.

Is there any value-added service?

Yes, there are two value-added services you'd be able to get with Allianz HealthInsured.

The first one is the International Second Medical Opinion which gives you as the Life Assured access to top experts from around the world to facilitate medical decision-making.

A licensed doctor will be assigned to conduct a thorough clinical conversation, collect all available medical information and identify top experts to provide an independent assessment.

Apart from that, you can also enjoy the Allianz Care@Home which is a monitoring service attended by medical professionals should you require it. However, this service is limited to qualified illnesses only.

How do premiums work?

The Allianz medical card comes with four (4) plans for you to choose from, making it available for everyone who needs medical coverage.

This way you can decide how much you want to be covered and the premiums that you are willing to pay.

Your premium will be based on individual factors such as age and gender. This plan gives you the freedom to customise your coverage and premium to suit your protection needs and budget.

What are deductible options?

Other than the zero deductible plan, you have the flexibility to choose from three deductible levels with a Retirement Option at 60 years old.

  • Level 1: RM5,000
  • Level 2: RM10,000
  • Level 3: RM30,000

Plans with a deductible can be utilised as an additional layer of protection on top of the coverage provided by existing insurance coverage. It also provides you with up to 70% savings on the Cost of Insurance.

Who can apply for Allianz HealthInsured?

One of the perks of the Allianz HealthInsured medical card is that it has a high entry age, where you can apply for the policy as young as 15 days old, up to 70 years old.

Please note that if you choose to go with the Retirement Option at 60 years old, the maximum entry age is 59 years old.

Enjoy guaranteed renewable coverage term until the maximum age of 100 years.

Additionally, if you are a mother-to-be, you can gift your preborn child early protection from 13 weeks to 36 weeks into pregnancy with similar guaranteed renewable coverage terms up to 100 years old.
