- Immediately contact Prudential BSN Takaful to inform them of your encounter for them to facilitate your claim and assist you wherever possible.
- There are a few forms that you'll have to complete as an evidence of your claim:
b) Attending Physician's Statement - to be completed by your doctor
- In addition of the above, make sure you have all the documents as required in the submission checklist to get you going. Here are some of them, just to list a few:
a) Medical Report
b) Medical Bills
c) Original Payment Receipts
- Once everything is good to go, deliver them over to any one of the Prudential BSN Takaful branches and they'll call you back in 10 working days with a result.
