How to Claim

Who do I contact if I have a question or emergency?

Prudential Customer Care Hotline

Hate going through complicated procedure whenever you're requesting for a reimbursement? Not anymore with Prudential's hassle-free claim process!

Panel Hospital

Prudential's medical card is the key to your speedy admission. Show it to the hospital attendant upon hospitalisation and you'll only be required to sign a Declaration & Authorisation form.

Forget to bring your medical card? No problem, giving them your IC number is equally sufficient.

Next, put a deposit upfront to provide for non-medical related items fees only if the hospital requests for it.That's all to get you admitted, pretty simple huh!

Upon discharge, check your bill and sign on it after you've paid the charges that are out of your policy's coverage.

Non-Panel Hospital

In this case, you need to pay the medical bills first and claim for reimbursement. Fret not, it can be done in just a few simple extra steps.

You have three options to get a claim form from Prudential:

  1. Through your servicing agent
  2. Contact their customer service at 02 21160228
  3. or send an email to [email protected]

Get ready the necessary documents such as original bills, original receipt and medical report and attach it with your completed claim form.

Deliver them to your nearest Prudential branch or pass it to your agent.

That's it! Your whole claim process is done and Prudential will notify you of the result.
