Notify Tokio Marine of the incident that's resulting in a claim within 30 days from discharge. Provide them with details such as your policy number, name of claimant, hospital name and admission date.
A claim form is essential to substantiate your claim, complete the relevant fields and have your attending doctor to fill up the last part of the form.
Also, they'll need some documents from you:
- Medical report
- Original itemised bill
- Original receipt
- Referral letter for pre-hospitalisation tests and consultation if any
