How to Claim

Who do I contact if I have a question or emergency?

Before you claim at Kurnia, make sure that you have or are admitted to a Kurnia Insurance Panel Hospital or clinic, check out the one closest to you, or give them a call to find the one most accessible, at their 24-Hour helpline at 1-800-88-3833.

Aside from the Claim Form, you will also need some supporting documents, such as the following.

For Accidental Death:

  • Completed Claim Form
  • Death Certificate
  • Post Mortem Report
  • Burial Certificate
  • Police Report and copy of driving license (if involved in a motor vehicle accident)
  • Letter of Administration (claim > RM100k) and Statutory Declaration from next of kin (claim < RM100k)
  • Sijil Faraid for Muslim
  • Nominee relationship proof (e.g, Birth Certificate, Marriage Certificate)
  • IC copy of the insured

For Permanent Disablement:

  • Completed Claim Form
  • Police Report and copy of driving license (if involved in a motor vehicle accident)
  • Medical Report, including a comment on the percentage of disability
  • Original photographs of the Insured Person/Claimant (full and close-up on the injury and disability)
  • IC copy of the insured

For Medical Expenses:

  • Completed Claim Form
  • Medical Report
  • Original Medical Bills and receipts
  • Police Report and copy of driving license (if involved in a motor vehicle accident)
  • IC copy of the insured

For Hospital Income:

  • Completed Claim Form
  • Medical Report
  • Copy of Inpatient hospitalisation invoice

For Weekly Benefits:

Once you have all of these things together, download and complete a claim form from Kurnia. They should send you one via fax or email.

For all the services covered by Kurnia, you will receive an offer letter and a discharge voucher for the amount payable. Get the discharge letter signed and delivered back to Kurnia for your claims to be released.

What is this Perfect 10 Plus insurance?

Perfect 10 Plus is a personal accident insurance plan that provides financial protection if you suffer from an injury, disability, or death caused solely by an accident. It's available for both individuals and families.

What does this personal accident plan cover?

The core coverage includes a lump sum payment for Accidental Death and Permanent Disablement. Beyond that, it also covers Medical Expenses from accidents, including treatment for Dengue, Malaria, or Chikungunya. 

It provides a weekly Hospital Income if you are hospitalised due to an accident, which is doubled if you're overseas.

What are the additional benefits of this insurance?

This plan has several "plus" benefits:

  • Personal Liability: Covers you up to RM100,000 if you're legally liable for accidentally injuring someone or damaging their property (excluding the US and Canada).
  • Financial Shield: Pays off your outstanding credit card, housing, or car loan balances (up to the limit) if you suffer Total Permanent Disablement.
  • Kidnap Cover: Provides a benefit of RM25,000 for expenses or ransom related to a kidnapping.
  • Snatch Theft: An additional benefit is paid if you suffer death or total permanent disablement due to a robbery or snatch theft.
  • Medical Evacuation: Covers the cost of emergency medical evacuation and repatriation if you have a critical medical condition from an accident while overseas.

Who can apply for this personal accident plan?

You can apply if you are a Malaysian, a Permanent Resident, or a Work Permit Holder (or their spouse/child) currently residing in Malaysia. 

The entry age for adults (proposer/spouse) is from 16 to 65 years old, and it's renewable up to age 70. Children can be covered from 30 days old.

Is there anything else I should know?

Yes, the plan has a Renewal Bonus. If you don't make a claim for Permanent Disablement, your coverage for Accidental Death and Permanent Disablement will increase by 25% each year, up to a maximum of 50%. 

It also extends to cover many specific situations, including terrorism, food poisoning (if hospitalised), and injuries while motorcycling.

Will I get a refund for any cancellations?

Yes. You can cancel your policy with 7 days' written notice. If you haven't made any claims, you'll be entitled to a partial refund of your premium based on the company's "short-period rates" for the time you were covered.

What documents do I need to renew online?

There are no specific documents for online renewal. However, for any changes, it is important that you inform Kurnia of any changes in your contact details, occupation, or personal pursuits, as this could affect your risk profile. For renewal, you would typically just need your policy details and a payment method.
