Aside from the Claim Form, you will also need some supporting documents such as the following.
For Accidental Death:
- Completed Claim Form
- Death Certificate
- Post Mortem Report
- Burial Certificate
- Police Report and copy of driving license (if involved in a motor vehicle accident)
- Letter of Administration (claim > RM100k) and Statutory Declaration from next of kin (claim < RM100k)
- Sijil Faraid for Muslim
- Nominee relationship proof (e.g: Birth Certificate, Marriage Certificate)
- IC copy of the insured
For Permanent Disablement:
- Completed Claim Form
- Police Report and copy of driving license (if involved in a motor vehicle accident)
- Medical Report including comment on the percentage of disability
- Original photographs of the Insured Person/Claimant (full and close up on the injury and disability)
- IC copy of the insured
For Medical Expenses:
- Completed Claim Form
- Medical Report
- Original Medical Bills and receipts
- Police Report and copy of driving license (if involved in a motor vehicle accident)
- IC copy of the insured
For Hospital Income:
- Completed Claim Form

- Copy of In-patient hospitalization invoice

For Weekly Benefits:

Once you have all of these things together, download and complete a claim form from Kurnia, they should send you one via fax or email.
For all the services covered by Kurnia, you will receive an offer letter and a discharge voucher on the amount payable. Get the discharge letter signed and delivered back to Kurnia for your claims to be released.
