What’s the point of having an insurance protection but clueless when it comes to submitting claim? Don’t be ignorant of your own safety and follow these simple steps:
- Notify Liberty Insurance within 30 days from the date of death. Then, you will be given a claim form and are required to fill it up.
- Also, prepare the necessary documents as a proof of your claim:
- For Death Claim: Certified copy of death certificate, copy of policyholder's IC and policy, burial permit, police report, post-mortem report (compulsory to non-Muslims).
- For Permanent Disablement Claim: Police report, and physician report and status report on the disability (both from the specialist)
