Before you go on to make a claim with Tokio Marine, you will first need give them a call to notify them of the incident that took place and after you have made a police report, if necessary, and get them to send you the claim form you need.
You will be required to put together a list of documents to deliver along with your completed claim form, and they will vary depending on the type of claim you are making.
For accident claims;
- Completed Claim Form
- Medical Report or Specialist Report as appropriate
- Original Medical Bills/Receipts
- Letter of Administration as applicable
- Written statement about the incident
- Copy of your NRIC
For fatal death claims;
- Police Report
- Post Mortem Report
- Death Certificate
- Burial Certificate
- Letter of Employment
- Last drawn salary slip prior to accident
- Copy of Deceased’s Identity Card/Driving License
