Just follow these steps to nominate a family member or legal relation to receive the benefits from your Group Term Life policy. You'll also get the nomination form in your mailbox with your Zurich policy.

- Fill up the form and details of your nominee(s)
- Drop off your signed nomination form at a Zurich Branch near you, or post your form to the following address:

How do I make a claim for Zurich Group Term Life?
To make a claim you can write to Zurich Call Centre and attach the required documents below.
- Completed Claimants Statement Form
- Certified copy of the Policy Holder's death certificate
- Certified copy of the Policy Holder's Identity Card
