What are the documents I should prepare to make an application?

If you are a salaried employee, please prepare the following documents.

  • Copy of your IC (both sides) and the IC of your Supplementary Card Holder if any
  • Latest 3 months payslips; or
  • Form BE; or
  • EA Form; or
  • Latest 6 months' payslips for commission earners

If you are self-employed, please prepare the following documents.

  • Business Registration Certificate; or
  • Copy of Form 9/24/49; and
  • Latest 6 months' company bank statement
