  1. Allianz Care Hospital & Surgical Claim Form - the discharge section to be filled by your doctor.
  2. Medical report
  3. Original bills and hospital receipts
  4. Police report or other supporting documents if applicable

Once you've got your documents ready, find an Allianz branch near you to submit your claims. If you have any questions about your health insurance cover you can also contact Allianz's customer service at 1 300 88 1028.

What is Allianz Booster Care medical card?

Allianz Booster Care medical card is a top-up medical insurance with complete healthcare benefits for when you're in or out of the hospital. 

What does it mean by a top-up plan? 

You may already have a personal health insurance or employee medical benefits, but this Allianz insurance can have you covered for the more major illnesses most conventional plans don't provide!

With your Allianz medical card, you will get extra protection for hospitalisation, surgical expenses, ambulance fees, and even home nursing care. 

You can decide on different deductible options to suit your budget, and also the flexibility to choose your annual and lifetime limits with 5 different Allianz Booster Care plans.

How much do I need to pay Allianz?

The premiums may vary depending on your age, occupation, and also the chosen plan. 

You will pay a lower premium rate if you choose a higher deductible amount. 

However, Allianz Booster Care is not available for those working as drivers, firemen, quarrymen, armed forces personnel, navy personnel, seamen, and police.

How does medical insurance deductible work?

How the deductible works is, that when you choose your medical insurance plan, be sure to choose a premium that you're comfortable with, as well as the deductible amount you are comfortable with. 

When you have a deductible, it means that only after you have paid the amount for the deductible, will the insurer cover your claims. 

Until then, when you have to make a claim to Allianz, you will need to pay the difference if the claim is more than your deductible, or the whole amount if it's less.

Do I get out-patient benefits with this medical insurance?

Yes, you can. Benefits include cancer treatment and kidney dialysis treatment coverage at Allianz panel hospitals. 

Allianz will pay for your cancer and kidney treatments up until your annual or lifetime limit. 

However, you need to pay the deductible amount before your medical insurance policy takes over!

Do I need to renew my Allianz medical card policy every year?

Yes, the duration of this plan is only for 1 year. You need to renew your policy annually to continue enjoying the healthcare benefits.

Can I cancel my Allianz Booster Care medical card policy?

You can cancel your medical insurance policy by returning it to Allianz within 15 days after receiving the policy. 

The Allianz insurance premiums that you've paid so far will be refunded to you in full if no claims have been made.

On the other hand, you can also cancel your Allianz Booster Care medical card policy after the cooling-off period. 

Just write a notice to Allianz and you may still get refunds from Allianz.
