STR: How To Get Free Medical Coverage If Your Household Earns Below RM5,000

Most families don’t realise it, but the government can cover your GP visits, medical equipment, and even critical illness if your household earns less than RM5,000 a month.

Programmes like mySalam, PeKa B40, and the MADANI Medical Scheme provide this protection at no cost, so you don’t have to worry about unexpected medical bills.

All you need to do is register for STR.

How To Register For STR

Your single STR application unlocks all three programmes. Register online or visit LHDN or UTC branches in person. You’ll need your MyKad and household details. Existing recipients can update their information via MySTR.

Once you’re approved for STR, you’re automatically enrolled in mySalam and the MADANI Medical Scheme.

To verify your enrollment and access your benefits, check mySalam (your password arrives via SMS), PeKa B40, or the MySejahtera app for the MADANI Medical Scheme.

Where To Get Care

To use the MADANI Medical Scheme, head to any registered panel GP clinic. You can easily find them by using the “Skim Perubatan MADANI” app, available on Google Play and the App Store, or through MySejahtera. You can also search for participating clinics online. Look out for the identifying stickers on the clinic entrance, and don’t forget to bring your MyKad for automatic verification during your visit.

For PeKa B40, you can access free health screenings at any Klinik Kesihatan or participating private clinics. These screenings cover common conditions like diabetes, hypertension, and cholesterol.

If you need to make a mySalam claim after being diagnosed with a critical illness or after hospitalisation, simply submit your Discharge Summary and Doctor’s Statement through the mySalam portal. Be sure to keep all relevant medical documents for your claim.

Common Questions About STR

What if my income changes?

If your household income rises above RM5,000, you’ll need to inform LHDN through the MySTR portal. Your eligibility will be reassessed during the annual review.

Do I need to reapply annually?

No. Once registered for STR, your coverage continues as long as you remain eligible. Just update your details annually through MySTR to maintain your benefits.

Can I use both government hospitals and panel clinics?

Yes. Use government hospitals and Klinik Kesihatan for routine care (RM1-5 per visit). Use your MADANI allowance at panel GP clinics for convenience when you need faster access to minor treatment.

What if I’m unemployed or working a low-wage job?

You still qualify as long as your household income is below RM5,000. These programmes specifically target families facing financial hardship, regardless of employment status.

Beyond Government Coverage

For families wanting additional protection, microinsurance through Bank Negara Malaysia’s Perlindungan Tenang initiative starts around RM70 annually. These policies cover accidental death and funeral expenses, risks not addressed by government programmes.

Parents with EPF savings can use Akaun 2 through i-Lindung to buy life or critical illness coverage for their children. Some basic plans cost just a few ringgit monthly. Starting 2026, the RM3,000 tax relief for life insurance extends to children’s policies.
